ADDICTION COUNSELOR

Job Summary

Provive Wellness of Wayne is looking for a passionate Addiction Counselor to provide comprehensive treatment and support to individuals struggling with substance use disorders. This role involves facilitating group and individual therapy sessions, conducting assessments, developing personalized treatment plans, and maintaining high-quality clinical documentation. As part of a multidisciplinary team, the counselor will play a key role in guiding clients through the recovery process and promoting long-term success.

Responsibilities

• Conduct intake assessments to evaluate client needs, substance use history, and co-occurring mental health conditions.
• Develop individualized treatment plans with measurable goals that align with evidence-based recovery approaches.
• Facilitate group therapy sessions focused on relapse prevention, coping skills, and emotional regulation.
• Provide one-on-one counseling to help clients navigate challenges, build resilience, and stay engaged in treatment.
• Maintain detailed and accurate progress notes, treatment plans, and discharge summaries in the electronic health record (EHR).
• Coordinate care with medical providers, case managers, and community resources to ensure a holistic treatment approach.
• Monitor client progress and adjust treatment plans accordingly to support sustained recovery.
• Educate clients and families on addiction, recovery strategies, and available support systems.
• Respond to crisis situations with appropriate intervention strategies.
• Stay current with best practices in addiction treatment through ongoing training and professional development.
