Summary of Job Functions:
- Accepting Cases from Illinois Department of Aging (CCU) and Illinois Department of Human Services, affiliated programs with Prudential and Private Sources
- Preparing service calendars
- Assisting clients with questions
- Monitor field staff compliance with mandatory in-service training
- Provide emotion support and encouragement to clients and homemakers
- Supervise field staff compliance with timesheets, Electronic Visit Verification (EVV) and attendance
- Acting as a liaison between client homemaker/CNA and CCU
- Maintain client personal files by documenting client contact and activities related to client services
- Provide daily activity reports to the Administrator
- Update client and field staff schedules
- Resolve family-client conflicts
- Maintain POC Compliance percentage of >90%
- Monitor EVV date in the Care When system on the daily basis
- Document all communication in the Care when system and homecare software pertaining to homemaker's schedules
- In-home supervisory visits.
Position Qualifications:
- A High School Diploma or General Education Diploma
- Demonstrated Management Skills, Combination of skills and experience that indicate a person has an ability to perform the supervisory activities
- Excellent Organizational and Communication Skills
- Certification of Completion of Illinois Department on Aging sponsored Community Care Program training
- Spanish Speaking is a plus.
