Main Responsibilities
* Provide comprehensive care to adult and geriatric patients, including diagnosing and managing acute and chronic illnesses
* Collaborate with physicians and other healthcare professionals to develop individualized treatment plans
* Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination
* Stay up-to-date with the latest clinical research and best practices in geriatric and adult care
* Participate in quality assurance and performance improvement activities to ensure continuous improvement in patient care
Requirements
* Master's degree in Adult or Family Nurse Practitioner Studies or Physician Assistant Studies from an accredited institution
* Minimum 2 years of experience as a Nurse Practitioner or Physician Assistant in geriatric or adult care
* Current licensure and certification as a Nurse Practitioner or Physician Assistant in the state of practice
* Board certification by AANP or ANCC in Adult, Family Practice, and Geriatrics or Adult-Gerontology Primary Care
* DEA license and NPI number
* Active Medicare provider number and eligibility to participate in the Medicare program
