The Registered Nurse Case Manager Role
The Registered Nurse Case Manager is responsible for screening referrals for appropriateness, performing admissions, and facilitating the transition of patients and families into the hospice program.
Main Responsibilities
- Coordinate the interdisciplinary care team for assigned patients and implement the plan of care.
- Provide primary nursing care for hospice patients in various settings, including home and skilled nursing facilities.
- Assume full responsibility for professional management of assigned hospice patients' care related to their terminal illness.
