Job Description
We are seeking a Licensed Practical Nurse (LPN) to provide basic skilled nursing care under the supervision of a Registered Nurse (RN). The ideal candidate will have proficient knowledge of home care and competency in nursing skills required for patient care.
- Monitor and document patient progress, reporting any changes to RN supervisors and the patient care team.
- Communicate effectively with patients, families, and healthcare team members to ensure quality care.
- Participate in center/agency surveys, mandatory training, case conferences, and staff meetings.
- Maintain current licensure, certification, and clinical records to meet federal, state, and agency regulations.
Requirements
To be successful in this role, you will need:
- A minimum of one year of practical nursing experience.
- A current and unrestricted LPN licensure in the state of practice.
- A valid driver's license and reliable transportation.
- Liability insurance to complete home visits.
- CPR certification.
Preferred Qualifications
Previous home health or hospice experience is preferred, or any equivalent combination of experience and training that provides the required knowledge, skills, and abilities.
