Job Description
The successful candidate will be responsible for providing and coordinating skilled nursing care under physician supervision, through the competent application of the nursing process. Key responsibilities include:
- Providing competent nursing care & interventions to home health patients according to state and industry standards;
- Supervising care provided by Home Health Aides, Licensed Practical Nurses (LPNs) assigned to the care team;
- Collaborating with the interdisciplinary team and attending physician to develop, integrate, and manage the patient care plan of care;
Requirements
- Acute care experience with adult or aging populations;
- Home health and/or skilled nursing experience;
- Nursing degree from an accredited college or university, or graduate of an accredited/approved RN program;
- Bachelor of Science in Nursing (BSN) preferred;
