Job Summary
The Occupational Therapist OT - Home Health Services role is responsible for providing evaluative, consultative, and treatment interventions to home health patients referred by a licensed physician in their place of residence.
Key Responsibilities:
- Assess, monitor, document, and report patient progress using required documents via clinician-preferred technology, within established timelines.
- Collaborate with the interdisciplinary team and attending physician to develop, integrate, and manage the patient care plan, making referrals to other disciplines as needed.
- Participate in center/agency surveys, mandatory in-services, case conferences, staff meetings, and programs as required.
- Maintain current licensure, certification, and clinical records to meet federal, state, and agency regulations and guidelines.
Requirements:
- Graduate of ACOTE-accredited school.
- Prior Home Health experience is preferable.
