We are seeking a compassionate and dedicated School Based Psychiatric Case Manager to join our team in the Struthers Schools. This role involves providing comprehensive mental health supports to ensure the well-being of our clients.
Primary Responsibilities:
Client Support:
· Monitoring client’s mental health symptoms and develop and implement individualized treatment plans.
· Offer solution focused sessions and psychoeducation with children/adolescents/ adults/families and/or groups.
· Maintain accurate and up-to-date records of client progress and interventions.
Collaboration:
· School based case managers will collaborate with teachers, administrators, and parents to create a supportive learning environment.
· Community based case managers will coordinate with community mental health providers to ensure seamless care.
Advocacy:
· Attend regular meetings to advocate/support client’s needs.
· Promote mental health awareness and stigma reduction.
Requirements
· Bachelor’s Degree in Psychology, Social Work, or Related Field Or,
· 3 years’ experience in a social service field
· Driver’s License
· License/Certification: LSW preferred
