Psychiatric Nurse Consulting & Therapy LLC is dedicated to providing compassionate and comprehensive mental health care to underserved populations in Mount Vernon. We primarily serve low-income individuals, those recently released from jail or the hospital, and members of the tribal community.
Position Overview:
We are seeking a compassionate and dedicated Case Manager to join our team. The ideal candidate will have a bachelor's degree in a related field and a strong commitment to supporting and uplifting individuals with mental health challenges.
Key Responsibilities:
- Assist clients in identifying and accessing community resources.
- Aid clients in filling out necessary paperwork and applications.
- Track and ensure clients are meeting their personal and treatment goals.
- Provide continuous support and encouragement to clients as they navigate their mental health journey.
- Coordinate with healthcare providers, social workers, and other professionals involved in the clients' care.
- Develop and implement individualized care plans tailored to each client's unique needs and circumstances.
- Conduct regular check-ins and follow-up meetings with clients to monitor progress and address any emerging needs.
- Advocate for clients’ rights and support them in resolving various issues.
- Maintain thorough and accurate records of client interactions, progress, and care plans.
**There is no expectation for transportation of clients.**
Qualifications:
- Bachelor’s degree in Social Work, Psychology, Human Services, or a related field.
- Demonstrated experience (either through work, internships, or volunteer work) with mental health populations.
- Strong understanding of community resources and how to access them.
- Excellent communication and interpersonal skills.
- Compassionate, patient, and dedicated to helping others.
- Proficiency in Microsoft Office and comfortable with various software applications.
- Ability to work independently as well as part of a multidisciplinary team.
Preferred Qualifications:
- Experience working in a case management or similar role.
- Familiarity with the local community and its resources.
- Bilingual abilities
Job Type: Full-time
Pay: From $22.00 per hour
Expected hours: 40 per week
