intervention, hospital placements and treatment, educational

Position Overview: We are currently seeking a Nurse
Practitioner to join our team and provide psychiatric care services at
our partner facilities. This role offers an opportunity for

Responsibilities:
- Provide on-site psychiatric assessments and medication
management to patients in skilled nursing, assisted living, and
other long-term care facilities.
- Consult,
educate, and coordinate care with facility staff, including
nursing home administrator, director of nursing, and director of
social services.
- Adhere to
established regulations, policies, and procedures while
exercising initiative and independent judgment.
