The Community Translational Research Institute (CTRI) CTRI brings together key institutions and individuals from the public and private sectors, including academic, public health and community medicine institutions, as well as neighborhood schools and community-based organizations for the prevention of chronic diseases. CTRI transforms population health practice through translation of prevention science into community-based programs and sustained practice and policy. The founding CTRI partners include leaders of the Claremont Graduate University School of Community and Global Health, the County of Riverside, the Inland Empire Health Plan, and the University of California Riverside School of Medicine.

CalAIM is part of the broader Medi-Cal Transformation and is designed to build a whole system, person-centered approach to care, in which clinical and social services are integrated to support people’s health and wellbeing throughout their lives. As a statewide, population health initiative, CalAIM will reach Medi-Cal members focusing particularly on advancing health equity by expanding resources available to populations and communities that have been historically under- resourced and under-served. CalAIM implementation involves scaling impactful services from initiatives statewide through Enhanced Care Management (ECM) and Community Supports.

Heluna Health invites applicants to apply for the position of ECM Lead Care Manager . The ECM Lead Care Manager manages and coordinates all enhance care management (ECM) and Community Support activities. Managing, supervising, mentoring, and coordinating training of ECM Lead Care Coordinators and ECM teams to meet the requirements of DHCS and ECM patient benefit. Coordinates performance improvement for the ECM and Community Support programs to ensure that all goals and objectives set by the organization are met. Support data analysis and research related to ECM and Community Support services.

This is a full time (40 hours per week), benefited position. Employment is provided by Heluna Health.

The pay rate for this role is $81,484.00 annually.

ESSENTIAL FUNCTIONS

  • Responsible for the comprehensive assessments that are inclusive of social, emotional, and medical needs.
  • Develops an individualized service plan in coordination with Managed Care Plans continuum of care as well as leverages relevant community resources as needed.
  • Administer intake questionnaires, assessments and other forms of tracking documentation as needed; track data for reporting, maintain case notes, and appropriate records and files.
  • Provide navigation, coaching, and counseling services as required.
  • Identify if individuals are connected to relevant services; if not, collaborate with community partners such as : clinics, public health, public assistance, psychiatry, mental health, etc. to ensure individuals are connected to eligible services.
  • Participate in the training and coordination of Community Health Workers (CHW) and Community Mental Health Workers (CMHW).
  • Supervision of health profession interns.
  • Assess targeted communities relative to relevant resources.
  • Provide a “warm hand-off” when individuals are connected to long-term services and providers
  • Maintain and interact in a culturally sensitive, respectful, and professional demeanor.
  • Attend community meetings with other service providers to share program information and coordinate services.
  • Comply with all policies and procedures guiding the work of this position and the program overall.
  • Attend training and meetings as required. Travel between various locations.
  • Reviews work for thoroughness, soundness of recommendations, and compliance with applicable standards, policies, procedures, and regulations.
  • Other duties assigned.

JOB QUALIFICATIONS

Education / Experience

  • A Master's degree or higher from an accredited college or university in a discipline related to the core business function of the department (Health Information Systems, Public Health, Public Policy, Psychology, Social Work, or a related field required.
  • Two (2) years in a highly responsible management experience in case management for underserved populations preferred.
  • Social work or direct service experience in community-based programs strongly preferred.
  • Demonstrated experience building collaboration and partnerships among diverse stakeholders, e.g., government, community organizations and businesses
  • Ability to coordinate day to day work among multidisciplinary teams (doctors, nurses, social workers, community health workers etc.) and service providers, including providing real-time problem solving and technical assistance as needed.
  • Must have knowledge and ability to implement the following evidence-based models : Harm Reduction, Strength-based Case Management and Motivational Interviewing.
  • Certificates / Licenses / Clearances

  • Licensed as a mental health clinician (LCSW, MFT, LPCC, PsyD, etc.).
  • A valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions.
  • Other Skills, Knowledge, and Abilities

  • Bilingual, bicultural in English / Spanish preferred
  • Proficient skill set in using an array of Microsoft Office Suite software programs such as Word, Excel, PowerPoint, Access, Adobe Reader, One Note, Outlook, Publisher, Teams, Outlook, Zoom etc.
  • Able to multi-task and set workload priorities for time sensitive projects / tasks.
  • Ability to problem solve and make recommendations to processes, policies, etc.
  • Able to communicate with all levels of personnel, e.g., written, verbal, in a professional and concise / clear manner; ability to work within a project team and / or independently.
  • Able to work in a very diverse environment and with diverse individuals.
  • Ability to be flexible in meeting changing work tasks and timelines; must be dependable and reliable.
  • Ability to independently manage and implement program activities in coordination with CTRI team.
  • Strong organizational skills, including an ability to manage multiple work projects simultaneously, track project details, and meet deadlines.
  • Strong technical skills with Microsoft excel and experience with database management (e.g., Electronic Health Record Systems) preferred.
  • Ability to attend meetings, provide training, technical assistance, and other job-related duties in locations throughout Southern California and have reliable transportation to carry out essential functions.
  • Ability to build supportive and respectful working relationships with underserved individuals that instills hope and promotes self-determination.
  • Must possess strong interpersonal skills.
  • Proven ability to work independently and as an effective and collaborative member of a team.
  • Strong community networking skills and ability to build resources and relationships that improve continuity of care.
