PROGRAM: Child First

JOB OVERVIEW:

The Family Resource Coordinator works in collaboration with their teammate, the Child First Clinician, to coordinate/provide community-based case management and mental health support services to the families on their caseload. The Family Resource Coordinator takes the lead in connecting the child and family with desired community-based services and supports while maintaining Child First’s reflective, relationship-based stance. The Family Resource Coordinator also enhances caregiver executive functioning skills (e.g., planning, organizing, managing time, focusing attention, regulating emotions, reflecting on progress) and engages in interactive play with young children in the home.

RESPONSIBILITIES:

  • Provide community resource expertise to the Child First team and families, including identifying and collaborating with community-based service providers and supports.
  • Engage with the Child First family and the Clinician in the collaborative family assessment process (i.e., use data from interviews, observations, interactions, and standardized measures to identify family strengths, needs, and challenges).
  • Promote family stabilization by identifying all needed and desired services, integrating service needs into the Child and Family Plan of Care, and addressing barriers to services as they arise.
  • Avert crises by assisting the Child First family in times of urgent need (e.g., eviction).
  • Enhance caregiver executive functioning skills (e.g., planning, organizing, managing time, focusing attention, regulating emotions, reflecting on progress) as needed and in consultation with the Clinician and Clinical Director.
  • Maintain a reflective lens when engaging with the caregiver in order to understand his/her motivation, needs, and possible barriers to new services and supports.
  • Collaborate closely with the Child First Clinician to meet the needs of Child First families.
  • Embrace the use of videotaping to enhance both therapeutic work with families and reflective clinical supervision.
  • Provide the identified child and/or other children in the family with an interactive, growth-promoting play experience.
  • Engage in weekly individual, team, and group reflective clinical supervision with the Clinical Director.
  • Engage actively in all aspects of the Child First Learning Collaborative, including in-person trainings, distance learning curriculum, and specialty trainings.
  • Track completion of all assessments and enter data into the appropriate database.
  • Keep all appropriate documentation for clinical accountability and reimbursement.
  • Participate in other clinical and administrative activities as appropriate.


Partnerships and Collaborative Relationships:

  • Establish and maintain relationships with provider organizations, regulatory and licensing representatives, and referral sources, establishing effective and productive working relationships while promoting PHMC.
  • Represent the organization before various stakeholder groups to maintain visibility.
  • Manage regulatory and accreditation contracts.
  • Serve on, participate in, and attend various committee meetings.


Contributing to the Team:

  • Participate in regular meetings with staff to ensure priorities are clear, coordination is good, and communication is open. Develop additional methods for ongoing effective communication.
  • Cooperate and communicate as a multi-discipline staff team member through formal meetings, informal consultations, and committee participation.
  • Model traits of a responsible team member by executing job responsibilities, open communication, and good follow through. Support team members to do the same.
  • Respond promptly to the concerns and interests of the clients, parents, guardians, funding entities, and other PHMC staff.

  • Demonstrate commitment to being trauma-informed and maintaining a therapeutic environment and community that promotes safety and nonviolence as the basis for all we do with clients and staff.
  • Demonstrate knowledge and utilization of trauma-informed practices including emotional intelligence, effective communication, understanding of the impact of trauma on development, strengths-based and person-centered practice, resilience focus, and commitment to growth and change.


Administrative Activities:

  • Ensure compliance with all PHMC’s policies and procedures, including confidentiality and acting as a mandated reporter in accordance with the law, policies, and procedures.
  • Implement all documentation requirements. Ensure that reports are complete, thorough, timely, and written in the prescribed format. Ensure that staff members do the same.
  • Other duties as assigned


SKILLS:

  • Ability to utilize collaborative, team-oriented approach
  • Knowledge and understanding of CASSP Principles
  • Behavior modification theory and application
  • Understanding of child and family development
  • Crisis intervention and management
  • Knowledge of DSM-V
  • Experience working with diverse populations
  • Strong computer skills in all Microsoft Office applications
  • Knowledge of early childhood development, parent education, parent-child relationships, and individual, family, and community-level risk factors (e.g., poverty, homelessness, maternal depression, domestic violence, substance abuse, teen parenthood)
  • Openness to learning, capacity for self-reflection, and eagerness to participate in reflective clinical supervision


QUALIFICATIONS:

Experience:

  • 1-3 years of experience in child and adolescent mental health services
  • 1-3 years of experience providing case management services in community settings
  • Knowledge of and experience with community-based services and supports in the service area is highly valued.
  • Experience working in homes and community-based settings with vulnerable populations of diverse cultures and ethnicities


Education: Bachelor’s degree required with at least 12 credit hours in psychology, sociology, social work, counseling, nursing, education, rehabilitation counseling, or theology

Other:

  • Clearances including PA criminal history, PA child abuse, FBI, and medical clearances
  • Valid driver’s license, access to a reliable vehicle with current registration, and evidence of current auto insurance required


SALARY:

  • Salary Grade: 16
  • Competitive salary based on experience
