POSITION FUNCTION: The Rapid Service Response Case Manager is responsible for providing necessary services to assigned families. The primary goals of the intervention are to keep family members safe, stabilize family functioning, and prevent the unnecessary out-of-home placement of youths. The Case Manager also assists in assuring continuous program compliance with external licensing and regulatory agencies and assists the Program Supervisor in program development, implementation, and evaluation. EDUCATION/EXPERIENCE REQUIREMENTS: Bachelor’s degree in social work or a related field At least three (3) years of experience working with children and families, preferably in the child welfare system ESSENTIAL JOB FUNCTIONS: Major highlights of the position requirements are noted below. Additional responsibilities may be assigned as required. The Case Manager has the following responsibilities while providing services for a caseload of six (6) families, in their homes and communities, in accordance with agency policies and procedures. Safety Assurance Responsibilities Make the first home visit within 24 hours of the referral, if the DHS worker is available within this time frame for the joint visit. Assess the safety of family members during each contact with the family. Immediately notify the Program Supervisor and the Department of Human Services when the safety of any family member cannot be assured. Provide 24-hour emergency “on-call” services to assigned families. Conduct crisis intervention sessions with families when needed. Arrange for emergency respite if necessary to ensure the safety of a youth in the family if appropriate. Assessment Responsibilities Be responsible for completing in-person home visitation two times per week, while ensuring all household children are seen at least once per week. Assist the family in identifying specific problems which adversely effect family functioning and threaten the break up of the family. Monitor the family’s ability to meet basic needs. Assist the family in developing a plan which will result in improved family functioning. Review weekly with family member’s progress made on the plan developed. Counseling Responsibilities Provide support and learning experiences (parenting skills counseling, stress management, communication skills, anger management, and child development). Emphasize the short-term nature of the program and assist the family with the process of disengagement. Treat all family members as primary consumers of the Case Manager’s services by responding to all complaints/concerns in a supportive and timely manner. Life Skills Development Responsibilities Develop structured skill teaching sessions (budgeting, housekeeping skills, hygiene, employment searches, meal planning, and organization skills. Provide other hard services (e.g. assisting with utility arrears) as needed and appropriate. Advocacy Responsibilities Assist the family in obtaining basic needs. Coordinate the provision of psychiatric, psychological, developmental, and/or medical evaluations. Engage with the local and community schools to reduce truancy. For all school-aged children, the provider must make at least one (1) visit within 15 business days from opening the case to the school to monitor the child’s school attendance and progress to ensure compliance with service goals. Service Management Responsibilities Assist the family in identifying and accessing community resources to meet current needs and ensure future need fulfillment. Keep the Department of Human Services informed of the family’s progress and difficulties verbally, in writing, and through joint visits. Update the Department of Human Services weekly via email and update the PFDS system every 6 days. Monitor school attendance and performance of all youths via contact with school officials and receipt of report cards. Ensure family members receive routine care as recommended by medical professionals. Maintain regular contact with other providers servicing the family. Keep other team members aware of progress and problems with assigned families. Administrative Responsibilities Maintain record keeping and documentation procedures on each assigned family according to program policies and procedures and regulatory criteria. Attend biweekly team meetings. Participate in weekly team meetings and weekly individual supervisory sessions. Participate in a minimum of forty (40) hours of training related to position responsibilities annually. Assist in the development of old and the revision of existing program policies and procedures. KNOWLEDGE, SKILLS, AND ABILITIES: Must have a valid driver’s license and insured vehicle Must be computer literate WORK SCHEDULE: Flexible forty (40) hour week Evening and/or weekend hours are required.
