What are the responsibilities and job description for the NAC position at Public Hospital District 3 of Grant County WA?
JOB SUMMARY
The Per Diem NAC will assist as needed in any of the following units: Assisted Living, Long-Term Care, ED, or Med/Surg.
SUMMARY OF DUTIES BY UNIT
Long-Term Care
- Assists all residents in daily schedule:
a. Assists in ADL’s to maintain independence to the individual resident’s potential. ADL’s include bathing, peri care, hair care, oral hygiene, skin care, ROM and ambulation, etc.
b. Keeps residents clean and dry at all times by following care plan. This includes toileting, turning, etc.
c. Provides clean linens and follows universal precautions for the protection of the residents and the rest of the staff.
d. Provides restorative assistance in addition to restorative CNA. This includes ambulation, ROM, and promoting self care when residents are able.
e. Gives lotion rubs, back rubs and provide other comfort measures for the residents. Positions residents for comfort as well as safety.
f. Dresses residents in the proper attire for the day including socks and shoes if available.
g. Offers fluids frequently during the day and night doing intake and output if ordered.
h. Passes nourishments to all residents as ordered. - Assists with meal times in a timely, courteous and proper manner. This includes serving, feeding, and monitoring. Also assists with cleaning areas at completion of meals.
Assisted Living
- Renders assistance to residents in need and documents on NAC flowsheets.
- Observes residents and reports changes in resident’s condition to the charge nurse.
- Follows daily schedule:
a. Assists in bathing residents in tub or shower according to the bath list.
b. Assists with combing hair, cutting nails (except of Diabetic residents) and assist in dental care individualized for each resident.
c. Assists with or provides pericare as per care plan.
d. Changes bed linens, if indicated.
e. Collects and bag soiled linens, dispose of waste in the proper receptacles.
f. Sees that residents have supplies: soap, toilet paper, etc.
Med/Surg
- Follows job assignments delegated by the Registered Nurse.
- Provides nursing assistance to patients/residents according to the guidelines addressed in the individual care plans and nursing orders.
- Assists all patients/residents in daily schedule:
a. Assists in ADL’s to maintain independence to the individual resident’s potential. ADL’s include bathing, peri care, hair care, oral hygiene, skin care, ROM and ambulation, etc.
b. Keeps patients/residents clean and dry at all times by following care plan. This includes toileting, turning, etc.
c. Provides restorative assistance. This includes ambulation, ROM, and promoting self-care when patients/residents are able.
d. Positions patients/residents for comfort as well as safety.
e. Dresses residents in the proper attire for the day including socks and shoes if available.
f. Offers fluids frequently during the day and night doing intake and output if ordered.
g. Passes nourishments to all residents as ordered.
h. Documents vital signs, weights and all care given in EPIC.
