Intake Coordinator

Puget Sound Home Health and Hospice is seeking an experienced full-time Hospice Intake Coordinator.

Requirements and Qualifications

• Education: High school diploma (healthcare-related degree preferred). Preference given to LPN candidates
• Experience: At least 2 years of Hospice, home health, or healthcare intake experience required.
• Technical: Proficiency in EMR systems, data entry, and insurance verification. (Medicare, Medicaid, and private insurance). Experience using Home Care Home Base is strongly preferred
• Communication: Strong verbal, written, and customer service skills.
• Organizational: Ability to multitask, prioritize, and collaborate in a fast-paced setting.
• Marketing & Relationships: Experience building and maintaining referral source relationships.

Job Summary:

• Process and verify hospice referrals, ensuring eligibility and insurance coverage.
• Obtain necessary consents, authorizations, and physician orders.
• Communicate with patients, families, and providers to coordinate care.
• Schedule initial visits and assign care teams efficiently.
• Maintain accurate records in the EMR system while ensuring compliance.
• Track and follow up on pending referrals for timely admissions.
• Support marketing efforts by building relationships with referral sources.
• Manage incoming calls, emails, and documentation related to patient intake.
