- Completes comprehensive in-home patient assessment
- Oversees comfort care and pain management
- Develops plan of care under primary care providers orders
- Provides direct patient care in accordance with the plan of care and scope of practice
- Provides accurate & timely documentation consistent with plan of care
- Participates in QAPI program to assure highest level of patient care
- Participates in interdisciplinary coordination of home health and hospice services
- Registered Nurse (RN) licensure
- Prior experience working with hospice patients is required
- Ability to work independently and demonstrate critical thinking
- Current BLS certification from AHA
- Be a licensed driver with an insured automobile in good working order
