Puget Sound Home Health and Hospice is seeking an experienced and caring full-time Hospice Registered Nurse Case Manager (RN) in Pierce County, Washington!
Benefits :
83,200 to $114,400 annually, DOE

Essential Duties
Completes comprehensive in-home patient assessment
Oversees comfort care and pain management
Develops plan of care under primary care providers orders
Provides direct patient care in accordance with the plan of care and scope of practice
Provides accurate & timely documentation consistent with plan of care
Participates in QAPI program to assure highest level of patient care
Participates in interdisciplinary coordination of home health and hospice services
Requirements and Qualifications
Registered Nurse (RN) licensure
Prior experience working with hospice patients is strongly preferred
Ability to work independently and demonstrate critical thinking
Current BLS certification from AHA
Be a licensed driver with an insured automobile in good working order
