About the Position
This is a full-time hourly position that would cover PSKC's patients in our Bremerton and Poulsbo clinics. The successful candidate will be responsible for working with patients and their families to address their psychosocial needs, develop treatment plans, and facilitate their adjustment to dialysis.
Responsibilities
- Assess patients' psychosocial needs and develop individualized treatment plans
- Facilitate patient education and support groups
- Collaborate with multidisciplinary teams to ensure comprehensive care
- Provide emotional support and counseling to patients and families
Requirements
- Master's degree in Social Work from an accredited college
- Licensed Independent Clinical Social Worker (LICSW) or Working towards LICSW with a Licensed Social Worker Associate Independent Clinical (LSWAIC)
- Prior renal experience preferred
- Excellent communication and interpersonal skills
