JOB SUMMARY: Responsible for the case management of program participants, providing support to the Sheriff Department staff, researching and preparing special reports, and recommending program policies and procedures as directed by the supervisor.
ESSENTIAL JOB FUNCTIONS:
- Provides case management services to program participants pre- and post-release.
- Screens and assesses program participants for the Reentry Program and Future Fit Program.
- Interviews program participants to determine what factors and roadblocks each individual faces in their recovery.
- Develops a release plan including housing and potentially Medically Assisted Treatment (MAT).
- Conducts follow-up interviews to ensure a smooth transition to the community.
- Assists participants with acquiring vital documents.
- Provides employment counseling, mock interview training, and employment leads.
- Aids with the transition from funded housing to independent living.
- Responds to both qualitative and quantitative questions; determines how to improve the program.
- Collects and analyzes data from program participants.
- Tracks participant progress in the program in its entirety; records outcomes.

WORKING CONDITIONS: Work is performed in a smoking-restricted office environment.
MINIMUM QUALIFICATIONS:
REQUIRED SKILLS, KNOWLEDGE, AND ABILITIES:

- Considerable knowledge of office procedures, practices, equipment, and terminology.
- Good knowledge of Sheriff Department policies and procedures.
- Ability to conduct research and compile information in a meaningful manner.
- Ability to establish and maintain working relationships with departmental staff.
- Ability to communicate effectively in writing.
- Ability to work independently and meet deadlines.
- Ability to attend work regularly and reliably.
- Skill in the operation of a computer utilizing word processing software.
PHYSICAL REQUIREMENTS:

- Visual acuity is needed for the use of the computer monitor.
- Digital dexterity is needed for the use of keyboard and dictation equipment.
- Ability to communicate effectively, both orally, with individuals and groups, and in writing.
EDUCATION AND EXPERIENCE:
Completion of high school or GED equivalency; considerable work experience in case management, administrative/clerical, public administration, business management, or a related area; or any equivalent combination of experience and training that provides the required skills, knowledge, and abilities.
