We are seeking a dedicated and experienced Director of Social Services to lead our team in providing comprehensive support and resources to our community. Responsibilities
Understanding the impact of social policies on residents' lives
Ability to advocate policies that promote social justice and equity
Engaging in policy analysis and development. Understanding the policies, procedures, and regulations of skilled nursing facilities.
Familiarity with the aging process, chronic illnesses, and disabilities common in long-term care.
Knowledge of available resources and support services for residents and families.
Working collaboratively with nurses, physicians, therapists, and other healthcare professionals.
Participating in interdisciplinary team meetings and care conferences.
Understanding the roles and responsibilities of other team members. advocating for residents' rights and needs within the facility and with external agencies.
Empowering residents to make informed decisions and participate in their care.
Promoting independence and dignity for residents. Ability to maintain professional boundaries and confidentiality.
Requirements

- Minimum of a Bachelor’s degree in a human-services related major, including sociology, gerontology, psychology, rehabilitation counseling or special education; AND
- One year in a healthcare setting where the individual is working as a supervised social worker who works directly with individuals
- SNF Discharge planning experience required.
- Proven experience in social services management or leadership roles.
- Strong knowledge of child welfare systems, workforce development initiatives, and addiction counseling practices.
- Familiarity with Section 8 housing programs and Medicare regulations is highly desirable.
- Excellent crisis management skills with the ability to remain calm under pressure
- Proficient in motivational interviewing techniques to support client engagement.
- Strong communication skills, both verbal and written, with the ability to interact effectively with diverse populations.

Pay: $80,000.00 - $85,000.00 per year
