Care Transition Coordinators are often involved in assisting patients through their care journey, from hospital or rehab to skilled care to home support care and beyond.

Qualifications and Requirements:
- Responsible for the complete referral process, including identifying home health needs, verification of benefits, and coordination of care.
- Coordinate with physicians, nurses, therapists, social workers, and discharge planners to ensure a smooth transition from one service to another.
- Cultivate new and existing relationships resulting in company growth.
- Initiate and expand referral network.
- Comply with all aspects of the company's compliance and HIPPA regulations.
- One year of home health, marketing, or case management experience or a combination of each.
- Must possess a valid driver's license.
Compensation: $30.00
