PurposeCare and its affiliate companies are looking for a qualified, dependable, honest, and reliable Wellness Coordinator. We have an immediate opening for a Wellness Coordinator who can coordinate wellness services provided by PurposeCare at independent living facilities.

Qualifications and Requirements:

• Coordinates and schedules welcome visits with a review of functions of the Wellness
  Center and maintains adequate supply of Wellness packets.
• Meets with other health care/wellness providers to establish relationships with local
  vendors
• Coordinating clients' schedules directly with staff or the staffing department.
• Participate in resident events (i.e., open houses, health fairs, lectures, etc.).
  
• Must hold an active CNA license in the state of Illinois
• Minimum of 2 years' experience in a health care setting, preferably home care
• Positive attitude, great communication skills, and ability to work independently
