Position Summary:

Putnam County HomeCare and Hospice is seeking a dynamic Hospice Business Development Representative & Clinical Liaison (RN) to serve as a key ambassador for our hospice program. This role drives the strategic growth of our hospice census through relationship-building, education, and timely patient transitions. The ideal candidate will combine clinical expertise with business acumen to increase referrals, ensure seamless patient transitions, and represent our services effectively within the community.

Key Responsibilities:

1. Business Development & Community Representation

  • Establish and maintain strong relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, home health agencies, and other key referral sources to drive hospice admissions.
  • Promote hospice services through targeted outreach, networking, professional presentations, and educational events.
  • Serve as the clinical expert in hospice care, effectively communicating service benefits to patients, families, and healthcare providers.
  • Represent Putnam County HomeCare and Hospice at community events, healthcare forums, and industry conferences to increase awareness and visibility of hospice services.

2. Scaled Growth of Hospice Census

  • Develop and execute a strategic growth plan focused on increasing hospice admissions and census in alignment with organizational goals.
  • Meet or exceed defined weekly, monthly, and quarterly growth targets for patient referrals, admissions, and census expansion.
  • Track, analyze, and report on referral patterns, conversion rates, and key performance metrics to ensure consistent growth.
  • Identify opportunities for market expansion and collaborate with leadership to refine strategies for increasing hospice penetration.
  • Partner with the Executive Director, Medical Director, and clinical teams to align business development efforts with operational capacity and high-quality patient care.

3. Coordination of Care & Patient Screening

  • Proactively identify and assess current palliative care patients for an appropriate and timely transition to hospice services.
  • Work closely with physicians, case managers, discharge planners, and home health teams to ensure early recognition of hospice eligibility.
  • Conduct clinical screenings and evaluations to determine hospice eligibility and facilitate smooth transitions of care.

4. Education & Training

  • Provide educational in-services and presentations to healthcare professionals, community groups, and families about hospice philosophy, eligibility, and benefits.
  • Serve as a resource for referral partners regarding hospice regulations, policies, and best practices.
  • Develop hospice education programs tailored to the needs of skilled nursing facilities, assisted living facilities, and physician offices.

5. Admission Coordination & Direct Patient Engagement

  • Facilitate hospice admissions by conducting comprehensive presentations of hospice services to patients and families.
  • Perform hospice admission assessments and complete required documentation efficiently and accurately.
  • Coordinate with the Hospice Medical Director and interdisciplinary team to ensure timely admissions and effective care planning.

6. Communication & Collaboration

  • Coordinate closely with the Hospice Medical Director, interdisciplinary team, and referral partners to optimize patient outcomes.
  • Act as a liaison between Putnam County HomeCare and Hospice and partner skilled nursing facilities, ensuring seamless collaboration and shared patient goals.
  • Provide regular reports and updates on growth strategies, referral success rates, and census projections to leadership.

Qualifications & Requirements:

  • Active RN license in Ohio (required).
  • Minimum 2 years of experience in hospice, palliative care, home health, or a related healthcare setting.
  • Proven experience in business development, marketing, or clinical liaison roles preferred.
  • Strong understanding of hospice regulations, eligibility criteria, and best practices.
  • Excellent communication, presentation, and relationship-building skills.
  • Ability to work independently and collaboratively in a fast-paced environment.
  • Proficiency in electronic medical records (EMR) systems and Microsoft Office Suite.
  • Valid driver’s license and reliable transportation for travel within the community.

Performance Expectations:

  • Meet or exceed weekly referral generation goals.
  • Maintain a steady increase in hospice admissions to achieve monthly and quarterly census growth targets.
  • Conduct a minimum number of weekly community outreach events, referral meetings, and patient screenings.
  • Provide weekly reports on referral patterns, conversion rates, and business development efforts.
  • Maintain a regular Monday through Friday work schedule, with flexible hours according to referral needs. No nights or weekends are mandated.

Job Type: Full-time

Pay: $67,000.00 - $80,000.00 per year
