PUYALLUP TRIBAL HEALTH AUTHORITY, located in Tacoma Washington is seeking an experienced full-time Pharmacist. The Pharmacists process and dispense medications, counsel patients on the use of prescription and over-the-counter medications, and advise the healthcare team about medication therapy.

ESSENTIAL FUNCTIONS:
- Counsel patients verbally on the appropriate use of prescription or over-the-counter medications and their contraindications, storage parameters, dosage, drug interactions, and potential side effects.
- Review new or refill prescriptions for appropriate indication, interpretation and accuracy of directions, dosage, dosage form, route of administration, quantity, potential drug interactions, drug incompatibilities, coverage by third-party payer, or determining if a generic form is applicable.
- Prepare or oversee the typing, preparation, labeling, and filling of new or refill prescriptions with the appropriate product, ingredient, dose, and dosage form including liquid medicines, ointments, creams, inhaled products, capsules, tablets, vaginal, rectal, or injectable products.
- Prepare prescription refill requests for provider authorization by reviewing and documenting subjective changes, biometric measurements, prescription adherence to therapy, completion of follow up with appointments or referrals, and/or completion of recommended labs.
- Maintain patient medication profile including review and updating of prescriptions, health conditions, drug allergies, drug intolerances, third-party payer information, address, phone number, and other demographic data.
- Prepare prescription authorization forms from third party payers or outside pharmacies refill requests for provider authorization by reviewing and documenting subjective chart notes, biometric measurements, drug history, prescription adherence to therapy, or completion of recommended labs.
- Assist in the recording and maintenance of records for general inventory, narcotic inventory, outdated or expired pharmaceuticals.
- Take and document patient histories relevant to drug therapy for over-the-counter products, medication consults, referrals, and/or provide advice on medication selection, medical equipment, or health care supplies for conditions including emergency contraception, diabetes, asthma, smoking cessation, or high blood pressure.
- Research and provide drug information on drugs to patients and/or members of health care team including dosing, uses, side effects, drug interactions, formulary coverage, retail or acquisition cost, or use in pregnancy or lactation.
- Perform drug utilization reviews addressing drug usage patterns for drugs with a potential for adverse reactions, overuse, misuse, or high cost.
- Attend pharmacy meetings and provide relevant feedback and suggestions for work flow improvement.
Requirements
- Current WA State Pharmacist license
- Five (5) years’ experience as a licensed Pharmacist
- Experience working with electronic health records systems
Salary : $137,280 - $160,680
