Volunteer Coaches. Existing PVSchools employees need to fill out our volunteer application to ensure
compliance and proper record keeping.

All district level volunteer / chaperone applicants need to fill out a Criminal Affidavit and be fingerprinted.
The Criminal Affidavit can be provided and notarized (at no cost) at the district office. Fingerprinting may
be accomplished by one of two methods :
