Key Responsibilities:
- Outreach members telephonically
- Answer incoming calls from members or company associates.
- Create Case, Create Tasks, Create Referrals in member’s chart in TruCare
- Complete screenings in TruCare
- Filter Excel Spreadsheet
- Utilize Outlook Calendar for daily planning.
- Document in TruCare
- Assist Member is downloading Wellframe App and Onboarding
- Educating members on Digital Case Management and Wellframe.
- Handle 80-100 calls per week (outbound and inbound calls)
- Complete screenings on all new members
- Document all member interactions and outreach in Trucare
- Onboard 5-10 members/week to Wellframe
- Skills-Referrals, care coordination, call center, Health care.
- Typing Test
- Medical – healthcare knowledge.
- Health care insurance
- Prior call center – care coordination
- HS Diploma GED.
