- Key Skills:CA LVN License .
- Acute Care background.
- Case Management experience.
- Discharge planning experience.
- Preference is for candidates coming with hospital setting experience.
- Will be working in SNF (Skilled Nursing Facility)
- Will be working as placement coordinator.
- Discharge Planning Experience In Hospital Setting.
