Key Responsibilities:
- SC will have 3 days of visits and 2 days of documentation weekly
- SC will have a caseload and work assigned tasks
- SC will complete H2060 assessment
- SC will fill put required forms for each assessment
- SC will make follow up calls.
- SC will complete documentation within 3 day turnaround time
- SC will stay organized and ensure work they do not fall behind
- SC will maintain a 90% or high in Audit scores.
- Skills-Social worker, LVN or Case manager.
- Social Service/ Social Work/ Care Management / Services Coordination.
- Nursing/ LVN/ Medical.
- Additional info from the HM: “Social Work, LVN, Medical background that would require at least a bachelor’s for social work and a license for LVN is preferred.
- I will review candidate with a strong medical background without the degree.”
- Note-Remote with travel (2 Days work from home, 3 days travel within 30 -40 miles radius in a week).
- Shift Time & Working Days: 8:00 AM to 04:30 PM CST, Mon to Friday.
