Key Responsibilities:
- RN must prep for the visit the morning of or afternoon before (Review of EMR and previous assessments).
- Be prepared to use critical thinking and problem-solving skills.
- Accurately document according to the provided documentation template which is a minimum of what is expected.
- Review all documentation and utilize Lenavi to identify inconsistencies that need to be corrected or validated.
- Complete and load supplemental tools and documents.
- Each day varies, nurse needs to be flexible with schedule and process changes, critical thinking and problem solving, prepared for travel, weather variability.
- Ability to be non-judgmental when visiting members’ homes and experiencing adverse events.
- Skills-MLTC, MAP UAS
- Complete 2-5 visits depending on the type of visit assigned.
- Visits have different time allotments assigned based on what is required during the visit.
- Individuals that have MLTC, MAP or Maximus experience.
- UAS RN
- MLTC, MAP or Maximus experience.
- Flexibility.
- Excellent written and verbal communication.
- Technology: outlook, Excel, Microsoft.
- AS
- Software Skills Required: Word, Outlook, Excel, Microsoft, Adobe, EMS.
- Required Certifications: RN, NY State Driver’s License.
- Organized.
- Education Preferred: BSN.
- Note-Location: F 2 F – Remote (Travel to the members home to conduct the visit.) Brooklyn or Bronx or Manhattan.
- Duration: 3 Months (contract to hire).
Shift Time & Working Days: 08:30 AM to 05:00 PM position (7.5 hours daily). Overtime may be required.
