What are the responsibilities and job description for the Assessment Counselor position at Pyramid Healthcare?

Summary: Responsible for facilitating all assessment services for program as outlined by facility standards and in accordance with clinical philosophy of program.
Essential Duties and Responsibilities:
- Conducts screenings and level of care assessments, using clinical knowledge and skill level and communicate the process with clients.
- Evaluates behavioral, emotional, cognitive, spiritual, and social needs of clients
- Collect demographic and historical information relevant to the LOCA
- Gather information for and complete precertification for all managed care clients when applicable.
- Facilitate admission process by communicating with appropriate clinical team member as required.
- Facilitates appropriate admissions to all levels of care
- Makes appropriate referrals to community providers and finds placement in appropriate setting for the client.
- Completes assessment, admission, and other paperwork for clients to in a timely manner. Including but not limited to Releases of Information (ROI) and other admission paperwork
- Manage admission pre-certification with funding sources when applicable.
- Collaborates with other team member, and departments as needed.
- Expected to provide coverage for other clinical team members
- Responsible for maintaining clinical record for clients with appropriate documentation of clinical services and client program engagement in ECR in accordance with program policy.
- Conducts brief individual therapy to engage patient in treatment and documents appropriately.
- Provides case management and linkage to alternate services if indicated.
- Develops an initial treatment plan.
- Completion of all required trainings as designated by the company and accreditation/licensing entities.
- Expected to provide and facilitate group therapy sessions as required.
- Other duties as assigned.

Required Qualifications
Education, Licensure, & Experience
Pennsylvania
- Non-ASAM aligned programs with Substance Use Disorder clients must meet one of the below requirements.
b) A Bachelor's Degree with the same educational requirements and 1 year of clinical experience.
c) An Associate's Degree with the same educational requirements and 2 years of clinical experience.
d) Full certification as an addictions counselor by a statewide certification body, which is a member of a National certification body or certification by another state government's substance abuse counseling certification board.
e) Current licensure in this Commonwealth as a registered nurse and a degree from an accredited school of nursing and 1 year of counseling experience.
f) Current licensure in this Commonwealth as a physician
g) Facilitating process group therapy, individual therapy and family therapy. Drug and Alcohol/Mental Health experience. Treatment planning experience.
- Programs treating clients in an OMHSAS Treatment Facility and/or Adult Residential Treatment Facility (RTF-A) must have one of the following:
b) Is licensed in a generally recognized clinical discipline that includes mental health clinical experience.
- ASAM aligned substance programs must meet one of the below requirements:
b) Master's Degree from an accredited college with a major in chemical dependency, psychology, social work, counseling and are working towards a Master's level licensure (Marriage and family therapy, social work, professional counseling), or certification by a state wide certification board (CAADC)
c) Bachelors Level with a CADC or are working towards a CADC with a minimum of one year of qualifying clinical experience.
d) High School Diploma or equilivant with a CAAC
e) Current licensure in this Commonwealth as a registered nurse and a degree from an accredited school of nursing and one year of qualifying clinical experience.
- ASAM aligned Co-Occurring enhanced programs must meet one of the below requirements:
b) Master's Degree from an accredited college with a major in chemical dependency, psychology, social work, counseling and are working towards a Master's level licensure (Marriage and family therapy, social work, professional counseling), or certification by a state wide certification board (CAADC)

Physical Demands
While performing the duties of this job, the employee is frequently required to talk or hear. The employee is regularly required to sit, use hands to finger, handle, or feel, and reach with hands and arms. The employee is occasionally required to climb stairs, stand, walk, stoop, twist, kneel, or crouch. The employee must occasionally lift and/ or move up to 25 pounds. The employee must seldom lift and/or move up to 50 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
Work Environment
The noise level in the work environment is usually moderately quiet & indoors. This position will require local travel. This position will be scheduled based on operational need & will be required to provide own transportation.
