Tapestry with Pyramid Healthcare is seeking a Full Time Case Manager to join their amazing team in Moscow, PA
Schedule: Monday - Friday Full Time w/ Benefits
Pay: $18- $23 per hour
Job Summary:
The Case Manager is responsible for the coordination of care for clients and their families in residential treatment. This includes providing individual and family case management services to support engagement in treatment, facilitating group sessions for education and skill development in collaboration with the clinical team, and managing admission, aftercare/discharge planning, and external provider coordination.
Pay & Benefits:
- Hourly Rate: $18 - $23 per hour (based on experience and qualifications)

Essential Duties and Responsibilities:
- Coordinate client engagement through appointments, aftercare plans, schedules, and meetings in collaboration with the treatment team.
- Communicate and coordinate with referral sources and outside providers, ensuring professional communication, assessing needs, and documenting contact from admission through the duration of treatment. This may include coordination for utilization review (UR), legal matters, and employment-related needs.
- Arrange and oversee transportation needs for clients.
- Provide individual case coordination sessions to support client progress.
- Assist in developing aftercare plans, including referrals and outreach to aftercare providers.
- Support clinical services by assisting with group sessions and maintaining weekly engagement with clients.
- Collaborate with the treatment team and maintain professional, clear communication with counselors and other staff members.
- Document all client, family, referral, and external provider interactions in compliance with program timelines.
- Work with the client navigation team to ensure seamless aftercare planning.
- Provide onsite support for Enhanced Outpatient Sessions (EOS) between clients and the Client Navigation team.
- Participate in mandatory corporate training, program training, and professional development opportunities.
- Perform other duties as assigned.

Required Qualifications:
Education, Licensure, & Experience:
- High School Diploma or equivalent required.
- One year of experience in a drug and alcohol or mental health setting preferred.
- Valid driver's license preferred.
- CPR/BLS certification preferred.
Job Knowledge, Skills, and Abilities:
- Strong oral and written communication and interpersonal skills.
- Sound clinical judgment and excellent problem-solving abilities.
- Ability to handle crisis situations and respond appropriately.
- Knowledge of federal and state standards as well as regulatory requirements.
