Our Assertive Community Treatment Teams (ACTT) provide a “hospital without walls” program that brings high intensity behavioral health services into the community to serve people who may not have access to the care they need. ACTT is a comprehensive team of professionals from many disciplines that provide tailored services to meet the client’s mental health, substance use, vocational and housing needs based on evidence-based treatment methods, comprehensive service planning, and tracking outcome measures. We work and contract with a variety of local and state agencies who conduct compliance and outcome studies for our clinical services to help our initiative for constant improvement with our outcomes for people with severe mental illness who are most at risk of psychiatric crisis and/or affected by other social determinants of health
Salary: $24.50 -$26 per hour
Schedule: Full Time 40 hours per week w/ benefits

Qualifications
Education & Experience
- Bachelor’s degree in Social Work, Counseling, or Psychology from an accredited institution required.
- Master’s degree in a Human Services discipline preferred.
- Must meet QP status according to 10A NCAC 27G.0104.
- 1-3 years of experience working with individuals with severe and persistent mental illness preferred.
Utilization Review
- Collect pertinent data related to patient care and treatment.
- Adhere to CARF and October Road CQI processes.
- Coordinate and obtain initial and ongoing approval from LME, ValueOptions, and third-party authorities.
- Develop person-centered treatment plans aligned with MH/SA treatment best practices.
- Ensure treatment plans address the needs of individuals, families, and consumers.
- Complete treatment plans in a timely manner while following CARF, DHHS, LME, and October Road CQI standards.
- Formulate appropriate post-discharge placement plans, ensuring continuity of care for consumers and families.
- Communicate pending and planned discharges to administrative staff.
- Complete all required discharge documentation, including summaries.
- Facilitate regularly scheduled treatment team meetings.
- Provide clinical expertise and actively engage with a multi-disciplinary treatment team.
- Conduct scheduled reviews of treatment plans with the treatment team.
- Provide written treatment plan updates for placement in clinical records.
- Obtain necessary consumer signatures on required forms.
- Maintain consistent communication with referral sources as appropriate.
- Provide updates on consumer progress.
- Develop and coordinate discharge plans in collaboration with referral sources.
- Prepare and conduct concurrent reviews for referral sources.
- Ensure timely and accurate completion of medical record documentation.
- Maintain records of all outside contacts as well as therapy sessions.
- Actively participate in at least one CQI team annually.
- Engage in the peer record review process.
- Conduct NCTOPPS data entry.
- Foster productive, effective, and professional relationships across all disciplines.
- Keep supervisors informed of significant issues and developments.
- Be receptive to feedback and supervision.
- Complete all required initial and annual training, including:
- Consumer Rights
- CQI
- Confidentiality
- Ethics
- CPR
- First Aid
- Provide transportation for clients to access community resources.
- Participate in emergency services duty on rotation, including commitment procedures, after-hours assessments, crisis planning, and hospital diversion.
- High moral and ethical values.
- Willingness to learn and grow professionally.
- Strong interpersonal skills to function effectively within a treatment team.
- Ability to maintain appropriate professional boundaries.
- Maturity in judgment and behavior.
- Valid driver’s license and ability to provide references and transcripts from an accredited college.
