QuadMed is looking for a Nurse Practitioner or Physician Assistant to join our team in our North Central Trust clinic in Muncie, Cambridge City and Yorktown, IN. This position will require travel to Muncie, Cambridge City and Yorktown clinics.

Schedule: 30hrs/wk

Job Requirements

• Completed master's degree level Nurse Practitioner or Physician Assistant program from an accredited college/university
• Minimum three (3) years primary care practice experience
• Ability to work independently
• Working with an Electronic Medical Record (EMR)
• Active and unrestricted applicable licensure
• Current CPR (BLS) certification or must be obtained within 60 days of hire
• NPI & DEA
• Current National Board Certification
