What Youll Do

As the Clinical Manager , you will play a vital role in ensuring that our patients receive the highest standard of care while fostering a positive and empowering workplace culture. You will :

• Lead and Inspire : Supervise and mentor a multidisciplinary team, promoting excellence and teamwork.
• Ensure Quality Care : Oversee the development and execution of patient care plans that meet the highest clinical standards.
• Drive Compliance : Stay ahead of California and federal home health regulations, ensuring the agency meets all requirements.
• Foster Growth : Encourage professional growth within your team through training, feedback, and development opportunities.
• Collaborate for Success : Partner with leadership to implement strategies that enhance patient satisfaction and operational efficiency.
• Problem-Solve with Compassion : Address challenges with a proactive mindset, ensuring seamless care delivery.
• Develop, implement, and evaluate : patient care plans to ensure quality outcomes.
• Assesses : the ability of the caregiver to meet the patients immediate needs upon admission and throughout care
• Monitor and analyze : clinical performance metrics to identify areas for improvement.
• Completes monthly chart : audits of 10% of patient census.
• Provides coding and OASIS : support to admitting clinicians.
• Participate / manage : in on-call rotation.
• Provides direct patient care : as staffing requires .
• Completes new employee : orientation for clinical staff.
• Maintains proficiency : with the agencys electronic medical record (Wellsky).

What Were Looking For

The ideal candidate is a compassionate and driven professional with a strong clinical background and leadership expertise. You have :

• Licensure : Active RN license in California (required).
• Experience : At least 3 years of clinical experience, with 1 years in a leadership or management role (home health experience preferred).
• Knowledge : Strong understanding of Medicare, Medicaid, and California home health regulations.
• Skills : Exceptional communication, problem-solving, and organizational abilities.
• Passion : A deep commitment to delivering patient-centered care and empowering your team to excel.
• Proficiency : in electronic medical records (EMR) system
• WellSky EMR Kinnser Management experience would be a plus.
