JOB FUNCTION: To provide services to pregnant women including referrals, education, monitoring of pregnancy and on-going support during pregnancy as well as during the first months of life, per Head Start regulations.
ESSENTIAL FUNCTIONS:
- Work closely with the Director of Health and Mental Health Services and staff to ensure that all Health Services for Early Head Start Expectant Families and infants (one year or younger) transitioning from the Expectant Families program are delivered effectively and efficiently.
- Assist Family Partnership staff to maintain accurate, confidential up to date health, dental and nutrition records for all EHS children coming from the Expectant Families program during the child’s first six months in the center-based EHS program as outlined in the Procedure Manual in compliance with Head Start Performance Standards, New Jersey Licensing and HIPPA regulations.
- Monitors and documents health/nutrition status of all EHS children coming from the Expectant Families program during the child’s first six months in the center-based EHS program in the program to ensure all screenings, assessments, immunizations are up to date and in compliance.
- Performs pertinent screenings for all EHS children coming from the Expectant Families program during the child’s first six months in the center-based EHS program to remain in compliance with Head Start Performance Standards
- In collaboration with family workers and the Center Director, review family intake materials and health updates for health/nutrition needs for children transitioning from the Expectant Families Program to EHS center-based.
- When appropriate, completes referrals for health, dental and nutrition issues of children; seeking outside resources as appropriate for children transitioning from Expectant Families program to EHS center based.
- Collaborate with Family Partnership staff to ensure that all children and families receive the required medical, dental and nutrition assessments and follow-up within the time requirements of Head Start Performance Standards and N.J. Licensing, for children transitioning from Expectant Mothers program to EHS center based.
- Advocates preventative health, dental, and nutrition care for all clients through established channels for available services to Head Start/Early Head Start families.
- Involved in regular, purposeful home visits for families or enrolled children as necessary.
- Direct Services for EHS Expectant Families Program:
- Acts as an advocate and liaison between Expectant Parents and service providers.
- Regular home visits with Expecting Parents according to Head Start Procedure.
- Completes the Community Family Partnership Agreement with all Expecting Parents, making referrals to community resources as needed with assistance from Community Family partnership staff.
- Assists Expecting Parents with transition from the Expecting Families program to child care as per parent’s preference within three months of child’s birth.
- Assists Expecting Parents obtain medical, dental, nutrition services per Head Start Performance Standards using established Community and Head Start resources.
- Addresses special dietary needs and/or requirements for EHS infants 1 year or younger for children transitioning from Expectant Families program to EHS center based.
- Coordinate with other social service agencies and community resources for EHS Expectant Families or for children transitioning from Expectant Families program to EHS center based.
- Collaborate with Family Partnership staff to assist families to access community resources.
- Collaborate with Family Partnership staff to compile and maintain an up-to-date community resource directory.
- Supervision for EHS services as it relates to the transitions and services during transitions for infants moving into Early Head Start home-based or center-based.
- Work with the Director of Health and Mental Health Services to develop Early Head Start policies and procedures in conjunction with other management, the Director of Compliance & Program Services, and the Executive Director.
- Coordinate with management for reporting suspected cases of child abuse and neglect with support and knowledge of the Director(s) of Operations and Director of Programs, as appropriate.
- On-going monitoring for EHS
- Submit all required statistical and narrative reports in a timely, accurate manner, including, but not limited to: reports and statistics to Local, State and Federal agencies as required, a monthly report (service area related statistics/issues, # of center visits, observations, issues, meetings, trainings, analysis of program issues along with suggestions).
- Assist in accurate tracking of all pertinent program information.
- Coordinate transition process of children and families coming into EHS and transitioning out of EHS.
- Attend parent and staff center meetings.
- Recruit community volunteers for the Health and Mental Health Services Advisory Committee with expertise in serving expectant and new families and participate in all Health and Mental Health Services Advisory Committee meetings.
- Perform all other assigned Head Start Duties as assigned.
QUALIFICATIONS:
- DULA or Graduate of an accredited school of Nursing (RN/LPN), preferred
- Early childhood health care experience, including knowledge of current childcare health requirements as mandated by local and state licensing bodies
- Ability to prepare and teach meaningful health education principles and practices to individuals of varying educational and socio-economic backgrounds.
- Minimum of two years management experience
- Must have a valid New Jersey driver’s license and access to a vehicle daily.
- Must submit to background/fingerprint check

Pay: $21.63 - $31.39 per hour
