Responsibilities
The Waiver Support Coordinator provides case management services for people with developmental disabilities, coordinating and monitoring services for adults and children. The Waiver Support Coordinator provides support to clients and their families, conducts monthly face-to-face visits, develops annual Support Plan, develops cost plan of services, and completes monthly case notes. On-site office work and off-site work visiting clients in different locations.
Qualifications
Bachelor's degree required and at least two years experience working with developmental disabilities, mental health, special education, or social work. Must speak English, have good written and oral skills, and transportation.
