What are the responsibilities and job description for the Parent Mentor/Case Manager position at Quality Connections Living Services?
Quality Connections Living Services is seeking highly motivated and compassionate individual(s) to join our team as a Parent Mentor Program Case Manager/System Navigator. In this role, you will work closely with the parents of children with developmental disabilities to provide guidance and support in accessing the resources and benefits available to them.
The essential functions include, but are not limited to the following:
- Review the case plan for each client, establish rapport, and build relationships with the family.
- Reference and follow the Case plan for each client when providing support.
- Maintain accurate and complete case tracking forms.
- Facilitate teleconferencing meetings for clients (Zoom/Teams/WebEx, etc.).
- Meet face-to-face with clients as appropriate and necessary.
- Connect clients to resources and services through QCLS’s various services and other community-based organizations.
- Maintain tracking records for each client, keeping track of time spent in session and notes summarizing the meetings.
- Submit documentation to department supervisor indicating billable hours, not exceeding the approved RCOC hours of service per month per client.
- Prepare quarterly reports and termination of service reports for each family served.
- Prepare data highlighting the numbers of families served and the numbers of resources given.
- Maintain an accurate resource database specifically tailored for the clients of Regional Center.
- Work closely with the department supervisor to complete expected tasks as assigned by either RCOC or by the Director of QCLS.
- Attend trainings and meetings as required of the employees of QCLS.
- Ability to accurately complete client records and maintain secure storage of personal information in office, on computer and when transporting.
- Able to meet timelines and deadlines.
- Be a positive representation of Quality Connections Living Services whenever in the community.
Requirements:
- At least 2 years of experience in a similar role, preferably working with families of children with developmental disabilities
- Excellent communication and interpersonal skills
- Strong organizational skills and attention to detail
- Ability to work independently and as part of a team
- Knowledge of community resources and services for families of children with developmental disabilities
- Reliable transportation
- Auto insurance that complies with the state policy concerning minimum car insurance coverage
- Valid driver’s license in good standing
- Successful clearance of local and/or state background checks
Job Types: Full-time, Part-time
Pay: $18/hr starting pay- $22/hr based on case load/experience
