Job Summary
We are looking to hire a well-trained & experienced PCA/CNA to join our team! You will work directly with patients, based upon their care plan. Providing personalized and attentive care by building genuine relationships with those you serve. One must be detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs. Strong verbal and written communication skills are needed to succeed in this role. Prior Home Health experience is a PLUS.

Responsibilities
- Assist patients with daily activities such as moving in and out of beds, baths, wheelchairs.
- Care for patients by changing bed linens, doing laundry, cleaning the home, or assisting with toileting/bathing.
- Maintain records of patient care, condition, progress, or problems to report and discuss observations with the RN Supervisor
- Build relationships with patients by conversing or reading aloud to help keep them mentally healthy and alert.
- Engage patients in exercises or other activities.
Qualifications.
- Experience as PCA /CNA
- High School Diploma or GED
- Driver’s license required.
- Background check
- Drug Test
- Must have dependable transportation.
