Quality Life Services is seeking a Social Services Director who is ready for a challenge and wants to make a difference in the lives of older adults. The main objective of this position is to coordinated internal and external social service counseling and education services.

As a Social Worker you'll be part of a collaborative team whose energy, communication and dedication are valued and rewarded.

Position Responsibilities (included, but not limited to)

  • Assess the psychosocial status of patients / families / caregivers related to the patient’s terminal illness and environment
  • Provide an assessment in the patient’s identified residence and assistance with this is not safe and another plan is required
  • Carry out social evaluations and plan interventions based on evaluation findings
  • Counsel patient / family / caregivers as needed in relationship to stress and other identified coping difficulties
  • Maintain clinical record on all patients referred to social work
  • Provide information and referral services for organization patients / families / caregivers regarding practical and environmental needs
  • Oversees and instructs medical social worker interns per specific college requirements
  • Maintain collaborative relationships with organization personnel to support patient care
  • Maintain and develop contacts with public and private agencies as resources for patient and personnel
  • Participate in the development of the plan of care and attend case conferences weekly
  • Assist physician and other team members in understanding significant social and emotional factors related to health problems and death / dying issues
  • Provide training, support and psychosocial development to the staff and referring facilities
  • Chair and maintain caregiver support groups internally and externally
  • Serve as member of the Hospice and Palliative Care Team, coordinating psychosocial care and activities
  • Participate in discharge planning when needed
  • Be available to support and counsel members of the Interdisciplinary Team as needed
  • Observe all Company policies and procedures.
  • Ensure compliance with all federal, state and local regulations.
  • Performs other related duties as directed by his/her supervisor.
  • coordination of outside appts for residents

Qualifications

  • Bachelor degree in Social Work
  • Minimum of one year’s experience in health care preferred
