RESPONSIBILITIES
- Certified Registered Nurse Practitioner (CRNP) is responsible for resident health and care within our skilled nursing/personal care facility
- Maintains compliance with PPHP Model of Care, Completion of Comprehensive Health Record (CHR), and reviewing appropriate HEDIS measures
- Assigning patient risk per PPHP policy
- Document any Transfer of Care (TOC)
- Review all testing, labs, and consults
- Complete walking rounds and communicate regarding resident concerns
- Report and log hospital admissions and any unplanned transfers
- Coordinate and complete employee physicals
REQUIREMENTS
- Completion of an accredited Nurse Practitioner program
- Current and unrestricted NP license in PA
