RESPONSIBILITIES

• Certified Registered Nurse Practitioner (CRNP) is responsible for resident health and care within our skilled nursing/personal care facility
• Maintains compliance with PPHP Model of Care, Completion of Comprehensive Health Record (CHR), and reviewing appropriate HEDIS measures
• Assigning patient risk per PPHP policy
• Document any Transfer of Care (TOC)
• Review all testing, labs, and consults
• Complete walking rounds and communicate regarding resident concerns
• Report and log hospital admissions and any unplanned transfers
• Coordinate and complete employee physicals

REQUIREMENTS

• Completion of an accredited Nurse Practitioner program

• Current and unrestricted NP license in PA
