RESPONSIBILITIES

• Psychiatric Mental Health Nurse Practitioner (PMHNP) is responsible for patient health and care within our skilled nursing/personal care facilities
• Maintains compliance with all regulatory and professional standards
• Collaborate patient needs, care and services with organizational and facility stakeholders
• Coordinate and complete patient encounters, treatment plans and meeting attendance and participation
• Assist with facility and organizational initiatives to achieve quality improvement objectives and outcomes

REQUIREMENTS

• Licensure as a Nurse Practitioner (NP) with appropriate state credentials

• Demonstrated experience in mental health care, especially with geriatric populations

• Strong interpersonal skills and a passion for improving quality of life for seniors
