Quality Staffing, Inc., also doing business as Goddard Mason Executive Search, a leading full-service employment agency matching qualified talent with some of the area's top companies, is looking for an experienced Community Health Program Coordinator to work through our agency as a contract employee with the Tennessee Department of Health (TN DOH) located in Cookeville, TN. This is a hybrid position working from 8:00 am to 5:00 pm Monday through Friday. The pay rate for this position is $31.00 per hour.
Community Health Program Coordinator Job Description:
The Community Health Improvement Plan (CHIP) Coordinator will provide subject matter expertise and support to County Health Councils who are working on their Community Health Assessment (CHA)/CHIP and will add capacity for Health Councils to implement programs, projects, or processes which will have the biggest impact in addressing their locally identified priorities. This position will enhance the Tennessee Department of Health's ability to support County Health Councils in collective impact approaches to improving health equity at the community level. **MUST be local to the Cookeville area!**
Community Health Program Coordinator Key Job Responsibilities will include:
- This position is responsible for professional public health program development and administrative work of considerable difficulty to include statewide responsibility for a large public health grant or program and will perform other work as required.
- Directly support the County Health Councils and TDH with CHA/CHIP program development, implantation, and monitoring.
- Attend meetings of the County Health Council at the local and regional levels.
- Lead outreach and relationship-building with state and regional partners in support of CHA/CHIP goals (TDMHSAS Regional Policy and Planning Councils, for example).
- Coordinate and assist with the implementation of initiatives related to the Community Health Assessment and Community Health Improvement Plan.
- Provide subject matter expertise regarding the implementation of the Community Health Assessment and Community Health Improvement Plan.
- Promote ongoing education and technical assistance on the CHA/CHIP requirements as outlined by the County Health Council and Tennessee Department of Health.
- Participate on the Community of Practice Advisory Board and serve as an ambassador for the Community of Practice.
- Provide support for new initiatives, including the Tennessee Vitality Toolkit, which can improve the execution of current initiatives.
- Consult and provide strategic advice to County Health Councils as necessary.
- Engage stakeholders in the CHA/CHIP programs, goals and objectives.
- Communicate success stories and accomplishments to the OSI team whenever applicable.
- Seek out and participate in professional development opportunities which arise.
Education and Qualification Requirements for Community Health Program Coordinator position:
- Graduation from an accredited college or university with a bachelor's degree and experience equivalent to substantial (five or more years of) full-time increasingly responsible professional health program work including, at least, two years of full-time supervisory work or statewide program oversite.
