Foster Care Coordinator

Need to urgently hire 1 Full-Time Treatment Coordinators in the Pleasant Grove location. Starting at $17 per hour plus medical insurance, paid time off, 32 cents per mile driven reimbursement, and $50 per month cell phone stipend. Starting wage is negotiable based on related field experience. Wage is negotiable based on experience and qualifications.

1. Supervise and train Trackers as they monitor clients in community, school, and employment settings. Monitor Trackers by reviewing all documentation of services. Provide scheduling, training, role modeling and the implementation of disciplinary actions if needed. Supervise and monitor clients in the community, school, and employment settings. Monitor clients in a proctor home setting by coordinating, scheduling, and documenting behavior and PRN Tracker services.

2. Provide ongoing support, supervision, training, and monitoring of proctor homes. Provide quarterly and annual home evaluations. Arrange respite services for proctor families. Assist in the completion of proctor per diem payment reports.

3. Assist APRN in the planning, scheduling, and preparation for medication management and psychiatric evaluations. Attend and report at scheduled medication management appointments. Provide quality assurance for the delivery and accuracy of all medications.

4. Assist in implementation of treatment plans by following the structure and expectations of the program, to include monitoring social skills and daily living skills instruction, providing a PRS group daily, coordination of medical and dental needs, education, employment, community service opportunities, and linkage to other community resources. Communicate with families of origin to report client progress, and coordinate home visits and other family contact.

5. Administer BestNotes documentation and assure all staff notes are complete and accurate.

6. Consult and communicate with clinical and administrative staff on a regular basis, to include attendance and participation in staff meetings and clinical staffing.

7. Contact Case Manager on a weekly basis to coordinate youth’s progress and provide quality assurance of services provided. Information shared should be indicative of treatment team decisions. Attend court reviews and ensure court obligations are met.

8. Maintain a safe, licensed, and insured vehicle for the transporting of youth to appointments, work, school, activities, etc. when needed. Coordinate transportation needs with proctor parents and educate and monitor clients’ use of the bus system.

9. Provide crisis intervention, conflict resolution, and behavior monitoring.

10. Document each client’s progress, interactions, and concerns on provided forms, such as: Tracking notes, information and incident reports, community service logs, or any other documentation as required. Complete paperwork in an accurate and timely manner.

11. Write weekly reports to relay the amount of progress youth makes in the program. Report is directly related to youth’s treatment plan.

12. Assist in the training and coordination support services for proctor/foster parents. Assist in the training and monitoring of homes quarterly and annually.

13. Provide back-up on-call services as required.

14. Perform other related duties as assigned.

Qualifications

• Knowledge of dysfunctional behavior, juvenile justice system, Division of Child and Family Services, and individualized counseling techniques.
• Excellent verbal and written communications skills.
• Uses good judgment and makes decisions in stressful situations
• Ability to interact positively with delinquent youth and de-escalate stressful situations.
• Demonstrate professionalism, maturity, and dependability.
• Manage time and efforts efficiently to manage case load, follow instructions, and complete designated tasks with little supervision.
• Must be 21 years old and pass a pre-employment drug screen and FBI Background check
• High School Diploma and at least one year professional employment.
• At least one year of experience/education working with Youth in Custody or in a related field.
• Must have reliable transportation with qualifying insurance coverage and meet the following driving requirements:
