Responsibilities:
- Evaluate patients' physical function and mobility
- Develop and implement individualized treatment plans
- Provide hands-on therapy to patients

Qualifications:
- New York State licensure as a Physical Therapist
- Experience working in long-term care or Skilled Nursing Facilities
