Queen Anne Manor is hiring for a Full-Time, Resident Care Coordinator/Wellness Coordinator position. We are looking for a dependable team player with a passion for top quality care and supervisory experience.
Job Summary:
The Resident Care Coordinator/Wellness Coordinator is responsible for the overall supervision, provision, and quality of nursing and care services in Assisted Living and Memory Care.
Responsible for the selection, training, discipline and supervision for all nursing and care related personnel.
Responsible for ensuring all residents receive exemplary care and favorable outcomes through the solid execution of daily quality standards.
Responsible for completing resident assessments and ensuring accuracy and compliance of resident care according to documented, Individualized Service Plans.
Responsible for coordinating resident services with external Care Providers such as physician, pharmacy, and hospice.
Responsible for monitoring task completion and documentation of care services and incident follow-up or investigation.
Ensures that any additional communication systems such as bath lists, laundry lists, daily task list, etc. reflect each resident's person-centered needs.
Works with the Executive Director and Regional Nursing Team to ensure compliance with State Requirements for the Department.
Responsible for the occupancy health of the community by working as a credible and collaborative member of the sales team alongside the Executive Director, Director of Sales, and others.
Qualifications include:
-Licensed through Washington Department of Health, such as CNA or LPN
-Computer proficiency. Must be comfortable utilizing technology systems on a daily basis to manage resident care and tracking.
-Strong interpersonal, communication and organizational skills. Strong supervisory experience with multiple levels of employees, with prior management level experience required.
