Job Description
We are seeking a skilled social worker to perform psychosocial assessments of patients and families/caregivers in our hospice program. This role involves providing timely and accurate documentation of patient services, counseling patients and their loved ones, and assisting with applications for government benefits.
Key Responsibilities:
- Perform psychosocial assessments of patients and families/caregivers
- Provide counseling and support to patients and their loved ones

Requirements:
- Social work degree from an accredited institution
- Current state licensure in social work (LSW, LISW, or LPSW-S)
- Minimum one year's experience in healthcare or hospice setting
