Schedule: 3 twelve-hour shifts per week, including rotating weekends from 8pm to 8am.

Your Role:
- Provide direct patient care as defined in the State Nurse Practice Act
- Implement current nursing practices based on comprehensive assessments and care plans
- Document patient services accurately and timely, reflecting the care plan
- Assess and educate patients and families/caregivers on diagnosis and self-care plans
- Participate in coordinating hospice services, reporting identified needs to the interdisciplinary group
- Manage pain and symptoms appropriately
- Evaluate patient responses to treatments/medications
Qualifications:
- Graduate of an accredited practical nurse or vocational nursing program
- Three years of nursing experience, preferably in community health, hospice, or medical/surgical settings
- Currently licensed as an LPN/LVN in the practicing state
- Maintain CPR Certification
- Demonstrate strong verbal and written communication, organization skills
- Valid driver's license with insured vehicle
- Understand the philosophy of hospice concept and the needs of terminally ill patients
