About the Role :

We are seeking a Case Manager to join our team at Epic High School – North. In this vital role, you will work closely with participants to identify barriers to success, develop individualized plans, and support students in achieving their personal, educational, and career goals.

As a Case Manager, you will :

Provide one-on-one counseling.

Connect participants with supportive services.

Monitor progress and advocate for students when needed.

Foster trust and accountability to empower students to build confidence and resilience.

This position reports directly to the Learn & Earn Program Director.

Key Responsibilities : Case Management

Develop individualized service plans with participants, outlining postsecondary goals and short-term / long-term objectives.

Conduct objective assessments to determine intervention levels using the MTSS tracker.

Administer QCH’s Self-Assessment survey biannually, creating SMART goals with participants.

Perform regular attendance outreach, including home visits.

Provide follow-up services for 35 program alumni annually for up to 12 months post-graduation.

Connect participants with QCH wraparound services and external support networks.

Academic Collaboration

Act as a liaison between students and teachers to monitor academic progress and facilitate structured meetings.

Regularly track and review grades and attendance with students.

Family & Community Engagement

Maintain regular communication with parents / guardians to support participants’ success.

Facilitate access to community resources and referrals as needed.

Administrative Responsibilities

Maintain accurate case files and document student interventions using DYCD Connect and Salesforce.

Participate in supervisory and program meetings, organization-wide meetings, and professional development sessions.

Attend parent-teacher conferences and other school-related events.

Program Development and Recruitment

Recruit 35 WIOA-eligible juniors and 35 WIOA-eligible seniors under the guidance of the Program Director.

Collaborate with teachers, guidance counselors, and DOE staff to identify and recruit at-risk students.

Refer participants, alumni, and families to QCH’s Pathways programming for additional support and resources.

Qualifications : Required :

Bachelor’s degree in psychology, education, social work, or a related field.

Minimum of 2 years of experience working with youth, ideally in a NYC high school setting.

Proficiency in computer applications and data entry systems.

Full-time availability with flexibility for school events, home visits, and training sessions.

Preferred Skills and Qualities :

Strong communication and interpersonal skills to engage youth, families, and community partners.

Attention to detail with experience in accurate data tracking using DYCD Connect and Salesforce.

Proficiency in Microsoft Office Suite.

Workshop facilitation experience for youth audiences.

Ability to manage multiple priorities, meet deadlines, and adapt to changing needs.

Experience building partnerships with local businesses, schools, and community organizations.

Passion for youth development and commitment to QCH’s mission of building inclusive communities.
