Position Title: Minimum Data Set (MDS) Coordinator
Reports to: Director of Nursing and Administrator

Position Summary:
The MDS Coordinator is a key member of the care team, responsible for managing and evaluating resident care, ensuring compliance with federal and state guidelines, and preparing essential reports for the national Medicare and Medicaid database. This role requires a balance of clinical expertise, leadership, and a strong grasp of data management to maintain accurate medical records and optimize reimbursement.
Key Responsibilities:
- Oversee the timely and accurate completion of MDS assessments as per RAI manual guidelines.
- Facilitate Care Conference Reviews at key intervals, including admission, quarterly, annually, and following any significant changes.
- Collaborate with the interdisciplinary team to review residents' status regularly, ensuring their continued skilled care needs are met.
- Manage the electronic transmission of resident assessments, adhering to CMS regulations.
- Ensure proper certification and compliance with Managed Medicare requirements.
- Maintain and update resident records, including admissions and discharges, and coordinate the appropriate closure of care plans.
- Participate in daily morning meetings and weekly Utilization Review meetings to stay informed on resident care plans.
- Stay up-to-date with continuing education through webinars, courses, and relevant resources.
- Assist with floor duties in urgent situations.
Qualifications:
- Active NYS Nursing License.
- Proficient in data collection, reporting, and analysis, with a focus on accuracy and compliance.
- Strong experience in developing resident care plans and conducting thorough assessments.
- Excellent communication, analytical, and problem-solving skills.
- Demonstrated leadership in clinical settings, with experience in long-term care preferred.
- Expertise in gerontological nursing to support the specific needs of elderly residents.
- Strong organizational skills and ability to manage multiple responsibilities effectively.
- At least one year of clinical supervisory or management experience preferred.
