About Us:
QUICKmed Urgent Care is currently seeking a highly motivated and skilled Social Worker or Counselor to join our team. This position plays a crucial role in supporting our mental health program, which serves both community clinics and school-based health centers.
Responsibilities:
- Provide counseling services to reduce barriers to comprehensive healthcare and address the basic needs of life for both students and patients.
- Foster strong relationships between school district staff, students, and school-based health centers.
- Collaborate with medical partners and community stakeholders to create a supportive network of resources for students, families, and patients.
- Work to improve access to mental health services and healthcare for individuals within the school districts and our community clinics.
- Conduct individual, group, or family therapy for both adults and children.
- Manage all incoming referrals and scheduling for QUICKmed mental health provider
- Answer incoming calls to QUICKmed mental health program
- Perform risk assessment and behavioral health screenings utilizing standardized tools on all incoming referrals for QUICKmed mental health provider
- Manage incoming referrals from community clinics and school-based health centers for students and patients with needs related to social determinants of health
- Help students, families and patients address related medical needs, including mental, vision, and oral health; social, developmental, and behavioral needs; and barriers to access (e.g., transportation, finances, language).
- Support collaboration and sharing information between the school and the School-Based Health Center.
- Perform delegated tasks (e.g., universal screening, scheduling referrals, gaining parental consent) to ensure that the direct care professionals can fully work of their role and license.
- Participate in ongoing standardized training, team meetings, and peer learning to build and maintain school-based health care coordination skills.

Qualifications:
- Masters Degree in Social work or Counseling
- Current active Ohio licensure as LISW or LPCC
- Excellent communication and interpersonal skills, with the ability to work effectively in a team environment
- Strong attention to detail and accuracy in documentation and record-keeping
- Proficient computer skills, including experience with EMR systems and Microsoft Office
- Phone skills
- Ability to multi-task and work efficiently
- Self-motivated and able to work independently
